| Medication Reconciliation |
OmniMD medical reconciliation is the process of comparing patients’ current medications to the list of their past medications to check for any duplication, dosage or transcription errors or omissions that could lead to adverse drug-to-drug interactions and serious medical complications.
The system reconciles present drugs to the past ones through patients’ eRx history from SureScripts, with in-bound CCD and/or CCR documents received from other providers and also with any scanned documentation available from patients or other providers who may have treated them in the past.
